The omega-3 fatty acids eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) have long been known to provide significant health benefits. More recently, a molecular mechanism has emerged that explains how they support a healthy inflammatory response. It’s also clear that to trigger this anti-inflammatory mechanism, a sufficient amount of bioavailable omega-3s must be consumed. This article will discuss a fascinating way omega-3s promote cardiovascular, immune, and cognitive health, among other benefits. It will also discuss the type of supplement your patients need to maximize these benefits.
Resolvins: A Molecular Rationale for Omega-3 Therapy
Resolvins are a family of endogenously generated compounds that produce Specialized Pro-resolving Mediators (SPMs), thereby suppressing inflammation.1 By fueling resolvins, the omega-3 fatty acids EPA and DHA act as biochemical substrates to produce SPMs.1 Resolvins originate while inflammation is being resolved in the body.1 While traditional anti-inflammatories block the origin of inflammation, resolvins derived from EPA and DHA actively shut off the inflammatory response and promote tissue healing.1
Resolvins have potent actions that may, at least in part, explain why omega-3 fatty acids have dramatic effects in immune and cardiovascular support, as well as other areas of health.2-4 One trial involving >11,300 patients found that supplementation with Omega-3 fatty acids reduced cardiovascular deaths by 45%.1
E‑series resolvins (RvE1–3) are generated from eicosapentaenoic acid (EPA),5 while D‑series resolvins (RvD1-6) are generated from docosahexaenoic acid (DHA) via lipoxygenase, cyclooxygenase, and cytochrome P450 pathways.4 It is interesting to note that aspirin can also generate the production of resolvins.1 Low-dose aspirin can acetylate the COX-2 enzyme, leading to the generation of “aspirin-triggered” resolvins.1 Aspirin‑acetylated COX‑2 can shift lipid metabolism toward aspirin-triggered resolvins from DHA and EPA, enhancing pro-resolving mediator formation in some contexts.1
Simply Taking Omega-3s May Not Increase Resolvins
Supplementing with EPA/DHA, especially at lower doses or with fish oil products that are not bioavailable, does not always translate into higher circulating resolvin levels, as key enzymes such as ALOX15 (15-lipoxygenase-1), which generate resolvin precursors, can be downregulated in some people, especially those with certain health conditions.6 Higher doses of EPA (4 grams/day) and DHA (2 grams/day) are often required to increase the plasma levels of resolvin precursors significantly.7,8
Health Benefits of Fueling Resolvins
Actively enhancing resolvin production through EPA/DHA intake is associated with several clinical benefits:
- Cardiovascular Health: Resolvins help stabilize atherosclerotic plaques and may lower the risk of stroke by reducing vascular inflammation.9
- Cognitive and Ocular Function: DHA-derived resolvins and protectins are highly concentrated in the brain and retina, supporting neuroprotection and healthy aging.10
- Joint and Muscle Support: In conditions like rheumatoid arthritis, increasing EPA and DHA can augment SPM precursor reserves, supporting a healthy inflammatory response in the joints.11
- Metabolic Health: Resolvins may act as “counter-regulators” to the persistent low-grade inflammation often found in adipose (fat) tissue in individuals with obesity.12
- Healthy Aging: The means by which omega-3 fatty acids reduce inflammaging, the term used to describe the relationship between age-related diseases and inflammation, may be connected to the ability to enhance resolvins.13
The Best Approach to Supplementing Omega-3s
After quantifying our patients’ omega-3 levels with a simple blood test, the next step to enhance resolvin levels is to supplement with a highly absorbable omega-3. High bioavailability increases the likelihood that enough EPA and DHA are delivered to the body to trigger resolvin production.
